About Solentra Health

Built on trust. Centered on people.

Solentra Health was created with a simple belief: good psychiatric care starts with taking the time to understand the person seeking help.

We want every patient and family who comes to Solentra to feel heard, respected, and supported. Our focus is not on rushing through appointments, but on building trust, making thoughtful decisions, and providing care with the attention and respect every person deserves.

Meet Dalia

Dalia Ascencio Yanez, APRN-FPA, PMHNP-BC, FNP-BC

Dalia Ascencio Yanez is a dual-certified Psychiatric-Mental Health Nurse Practitioner and Family Nurse Practitioner. She has been practicing nursing since 2009 and has worked as a nurse practitioner for more than 10 years. Her clinical background includes nursing experience in cardiac and emergency care, along with nurse practitioner experience in emergency medicine and urgent care. Over time, she developed a deep interest in psychiatric care and the important role mental health plays in overall well-being.

Her diverse clinical experience has shown her how closely physical health, mental health, relationships, responsibilities, and life circumstances are connected. It has shaped her belief that good care means looking beyond symptoms to understand the whole person.

After earning her Post-Master’s Certificate as a Psychiatric-Mental Health Nurse Practitioner from Southern Illinois University Edwardsville, Dalia founded Solentra Health to create a practice where patients have the time and space to discuss their concerns and participate in thoughtful, informed treatment decisions.

Her approach is grounded in clinical knowledge, open communication, and respect for each person’s experiences, questions, and goals.
